LIABILITY RELEASE FORM

This release is made a part of the Training Agreement.

By the signing hereof, I agree as follows; I have contracted with Wingswept Llewellins, aka Wingswept Llewellin Setters, to train my dog to Quail hunt. I hereby hold harmless for all time Wingswept Llewellin Setters and Deane F. Harris from any accidental harm or death to my dog while the process of training occurs. I am personally aware of the inherent dangers involved with taking hunting dogs afield and accept the risks involved. At such time as I myself try my dog in the field the following release of liability will apply.

In consideration of my being allowed to have access and the use of certain properties owned or controlled by Wingswept Llewellin Setters for bird hunting and /or related purposes, which properties are made available to me shall be undertaken by me at my own risk, and I acknowledge that I understand the risks involved in shooting birds over dogs.

Wingswept Llewellin Setters, Deane F. Harris, Virginia B. Harris, nor any person nor affiliated company or group of companies shall be liable for any claims, demands, injuries, deaths, damages, costs, fees, or fines of any nature or kind suffered by me or my property, or property used by me, for any actions or cause of actions of any nature suffered by me or my property.

I do hereby hold harmless, further expressly forever release, discharge, indemnify Wingswept Llewellin Setters, Deane F. Harris, et al, from all such claims, actions, causes of actions, demands, damages, injuries, costs, fines, whatsoever.
